Outpatient Program (OP)
The Outpatient Program (OP) at Nexus is designed for those who are transitioning from an intensive outpatient program, or for clients who need less intensive treatment and require a flexible schedule to accommodate work, school, or family obligations. At the OP level of care, we facilitate recovery in a supportive and structured environment, with clients participating in the program for two hours a day, one day a week.
At Nexus, we’ve made it our mission to provide each client with an individualized experience. Consequently, our top-notch clinical team has the ability to customize our program in any way to meet our clients’ needs. Treatment plans are designed by our clinical team, and are focused on giving clients both short-term, attainable goals while also establishing long-term goals. As we greatly value our clients’ input in regards to treatment, treatment plans are updated by our clinical team on a weekly basis with client feedback in mind.
If a client is in need of housing during the recovery process, we are also able to provide resources for sober living homes that are convenient to our facility.
Aspects of the Outpatient Program
Coordination of Medication Management: During this phase of treatment, we may refer you to a psychiatrist who can adjust or change medication, as needed. Medication management outside of treatment will also be discussed to ensure that any mental health conditions are properly treated once you transition out of our care
Individual therapy: In this final stage of treatment, you will still meet with your individual therapist on a weekly basis. At this point, sessions may focus on setting you up for success outside of treatment by identifying and working through any underlying issues that may impact maintaining sobriety outside of Nexus.
Group therapy: OP clients can still attend a variety of Process and 12-Step Groups that are offered on a daily basis. These groups continue to hold you accountable to your sobriety, address relapse prevention outside of our care, and help you maintain a sense of community and self-awareness that will follow you outside of treatment.
Family dynamic review: Before leaving Nexus, clients are encouraged to engage in a family dynamic review led by one of our trained family practitioners. This review is helpful, as familial support and honest communication is extremely beneficial as you transition out of treatment and back into your home and community.
Case management: As you prepare to transition out of our care, you will work with your case manager to put finishing touches on your Continuing Recovery Aftercare Plan. Your case manager is there to ensure that all necessary services and support systems are in place that will support you outside of treatment and contribute to your lifelong sobriety.
Community involvement: Even in this less intensive phase of treatment, we still encourage all members of Nexus to maintain involvement within the community. You may continue to take part in our internship program, attend social events, and maintain any Big Brother relationships you developed during the recovery process.
Budgeting & financial planning: At this point in care, we want our clients to leave Nexus with a set of life skills that support them outside of treatment. In addition to any further career or education assistance that may be needed, we also offer budgeting and financial planning to help clients understand and maintain financial health as well as physical health.
Transition & aftercare planning: During this phase of treatment, your Continuing Recovery Aftercare Plan is solidified and a transition plan put in place to ensure a smooth discharge process. We are able to provide referrals to any resources you may need to support you outside of treatment, including sober housing, medical care, obtaining a sober mentor, and getting involved with a local 12-step group.
