Our partial hospitalization program makes a difference. The PHP program at Nexus is designed for those who are new to the recovery process. At the PHP level of care, we are able to facilitate recovery in a supportive and structured environment, with clients participating in the Nexus program for a minimum of six hours a day, five days a week.

During this time, our goal in PHP is to create a foundation for our clients to carry with them through the recovery process and that will set them up for success as they transition back into their communities.

As a boutique recovery center with a small client to clinician ratio, we are able to provide each client with an individualized experience and can tailor our treatment program to meet our clients’ needs. All treatment plans are designed by our clinical team in conjunction with the respective client. Most often, these treatment plans focus on giving each client short-term, attainable goals while also identifying long-term goals that may extend beyond treatment.

If clients are in need of housing during the recovery process, we are also able to refer them to sober living options.

What Does a Typical Week in PHP Entail?

While we believe in a customized treatment experience for each client that walks through our doors, a basic outline of what a week in PHP includes is:

Sample Schedules

Everyone’s experience at Nexus Recovery is different. We do not believe in a one-size-fits-all approach. As a boutique treatment center we are able to customize an individual’s treatment experience based on their unique needs. However, we know it can be helpful to visualize what a typical week may look like for someone in the different phases of our program. Below you will see sample schedules for the PHP and as well as the IOP levels of outpatient care.

1 PHASE ONE (PHP)

The PHP level of care can last anywhere from 0-30 days. How long someone is in PHP depends upon their individual needs and the continued assessment of the clinical team.

Treatment takes place for 6 hours a day, 5 days a week and includes a variety of treatment modalities. This phase is designed to help lay a foundation for continued growth and success as a client moves through our program. Below is a sample schedule of what a week in our Partial Hospitalization (PHP) program can look like. However, it is important to note that this is just a sample and subject to change based on recommendations from the clinical team and individualized needs.

2 Phase two (IOP)

The Intensive Outpatient (IOP) level of care can last anywhere from 0-30 days and either follows the PHP level of care or, for some clients, can be a starting level of care.

The assessments conducted prior to admission into our program are reviewed by our clinical team and help determine what level of care a client will begin at. Clients in this phase are held to a higher standard. They are likely attending school, working, and/or more involved in their family life. Unlike in PHP, clients attend treatment for 3 or 4 hours per day 3 or 5 days per week depending on need.

Again, it is important to note that the schedules below for both our men’s and women’s IOP programs are meant to serve as samples only. An individual’s schedule changes based on their unique needs.

3 Phase Three (OP)

The Outpatient (OP) level of care is generally meant for clients who have been sober for an extended period of time.

Many of the clients in the outpatient level of care have already completed PHP and/or IOP. During this phase clients are attending treatment at Nexus for either one or two days per week and spending more time outside of the treatment facility pursuing or engaging in work, school, and/or familial obligations. Clients are able to reap the benefits of group and individual therapy, in addition to continued case management and peer support.

Countless studies show that the longer someone is engaged in a treatment program, the better the chance they have of staying sober long-term. While the time spent at Nexus is more minimal during this phase, the accountability and support that Nexus provides at the outpatient level of care is critical to our clients long term recovery and success.

4 Phase Four (Aftercare & Alumni Programming)

Once you are part of the Nexus Recovery Community you are part of our family for life.

Usually after someone has been involved in treatment for six months or more they enter our Aftercare and Alumni Programs. Clients in this phase have transitioned to fully independent living. While we will continue to communicate and assist clients in this phase as needed, these clients can confidently live life sober with minimal support from our clinical team. By this time, we have helped out clients learn the skills they need to successfully reintegrate into society, realize their potential, and find, meaning, purpose, and passion in their lives. Many clients choose to stay involved in therapy and our alumni services long after they leave our care. Clients will leave Nexus with established connections to their mind, body, and spirit, along with a clear set of tools and a support system in place as part of their relapse prevention plan.

Elements of PHP Treatment at Nexus Recovery

Our program is comprehensive, and incorporates elements that combat addiction from all angles. 

A thorough initial assessment: Upon entering PHP, members of our clinical team create a master treatment plan for each client. This treatment plan is designed specifically for you and is curated to meet your individual needs. At Nexus, we believe client input is important to the recovery process. Therefore, you and your treatment team will revisit this master treatment plan on a weekly basis and can make changes based on your feedback and progress.

Coordination with Psychiatrist: When you arrive at Nexus, we will coordinate with your psychiatrist or refer you to a psychiatrist to ensure that you will be seen within your first few days by a psychiatrist who specializes in addiction. This will provide information about any mental health conditions and identify the best possible way to address them in conjunction with your other treatment goals.

Individual therapy: Each week, you will meet with your individual therapist, providing an opportunity to explore underlying issues impacting your recovery. Therapy often focuses on developing the capacity for emotional self-regulation, which in turn, helps builds a strong foundation for lifelong recovery.

Family therapy: Multi-family education group is offered weekly. Clients’ attendance is required, and we encourage you to include your family members. If needed, private family and couple therapy sessions may also be available. With the support and assistance of a trained family practitioner, clients learn to communicate productively and honestly with family members.

Group therapy: Nexus offers a range of groups on a daily basis, including Process Groups, Multi-Family Group, Psycho-Education Groups, Anger Management, 12-Step Groups, and Experiential Groups.  Although the goals of each group may differ, all are designed to enhance knowledge and insight, communication skills, empathy, and self-awareness.

Case management: As part of your treatment team, you will be assigned a case manager who will assist with developing and monitoring your individualized treatment program. Your case manager also help you create an individualized aftercare plan, which may include referral to additional support and advocacy for employment services, social services, housing, transportation, and medical evaluation/treatment.

Psycho-Education groups: Psycho-Education groups address varied topics, all of which have been demonstrated to contribute to an understanding of addiction, relapse prevention, emotional self-regulation, and development of healthy interpersonal relationships.

Experiential groups: In addition to the talk-based therapeutic groups described previously, Nexus offers a range of complementary experiential groups. These may include holistic treatment modalities such as: Psychodrama, yoga, nutrition, meditation, and Rock to Recovery Music Therapy.

Adjunct holistic services: Nexus offers the integration of holistic therapies designed to enhance recovery. Some of the integrative treatment options at Nexus include acupuncture, spiritual counseling, neurofeedback, Somatic Work, and EMDR.

Community involvement: Substance abuse is often a disease of isolation. Becoming part of the Nexus community can strengthen social skills, interactivity, and decrease social anxiety. Group therapists at Nexus will model and promote healthy group interactions and encourage you to engage with others in an effective and positive manner.

Spiritual counseling: A valued component of your treatment at Nexus is engagement in spiritual counseling.  It’s worth has been well demonstrated in the recovery process; in fact, the foundation for the 12-Step model is its emphasis on the role of spirituality.  This is not to be confused with religious advocacy of any kind.  At Nexus, we do not emphasize or promote any particular religious belief system or observance. However, we are profoundly aware of the key role to be played an enrichment of each client’s spiritual life. Key elements of our spiritual counseling program may encompass increasing self-knowledge, connecting with one’s inner wisdom and life-force, developing self-compassion, building feelings of self-worth, exercising gratitude, and exploring one’s life purpose. Our therapists incorporate many of these spiritual elements into group and individual therapy sessions. In addition, we provide a weekly group with a trained spiritual counselor who specializes in spirituality as it relates to addiction and recovery.

Nutritional counseling: At Nexus, we recognize the profound role that nurturing the body can play in recovery. Substance abuse often results in a decrease in the levels of key neurotransmitters such as dopamine and serotonin.  This can cause depression, an increase in anxiety, and profound mood changes.  Dietary adjustment based on the new science of addiction nutrition can help mitigate these effects, improving mood and coping skills that aid in the recovery process. We will have a weekly group devoted to optimal nutritional health.

Career & educational goal setting: At Nexus, we encourage our clients to envision their lives beyond treatment. Consequently, part of our program includes helping clients identify career and/or educational goals and putting plans in place to make these goals achievable.

Family program: Addiction is not only an individual issue. Often, anxiety, uncertainty, and traumatic stress reactions are experienced by family members and significant others. Nexus is deeply committed to educating and supporting your family and significant others during the recovery process. We provide a weekly multi-family process and psychoeducation group, as well as the possibility of private family and couple therapy sessions.

Group outings: Part of our program is showing clients that they can have fun while sober. Consequently, we offer a variety of group outings which may include beach days, movie nights, surfing, and hiking.

Continuing recovery & aftercare planning: At Nexus, aftercare planning begins during day one of treatment and is a joint effort between you and your treatment team. Your Continuing Recovery Aftercare Plan will encompass a comprehensive set of recommendations for lifestyle strategies and support systems that will help you maintain your sobriety after you leave treatment.
